Important Points for CLIA Labs

Clinical Laboratory Improvement Amendments (CLIA) certified laboratories on campus are also subject to this inventory and must complete an inventory form from the ORCBS even if they do not have any of the materials listed on the form. The Centers for Disease Control and Prevention should be sending an official inventory form directly to each CLIA certified lab. This form contains that laboratory's designated username and password to be used when reporting the results of your inventory online. ORCBS is responsible for reporting their response, so please do not respond yourself. If you receive any forms from the CDC on the polio inventory, do not throw them out!
