INSTRUCTIONS
FOR FILLING OUT THE ONLINE REGISTRATION FORM
In order to
access the online registration form, you must first sign in using
your name and access code, which is also used for accessing safety
training records through the ORCBS website. Only Principal Investigators
may submit registration documents. If you are a new Principal Investigator
and your name is not on the drop-down menu or you are not a Principal
Investigator, call Jamie Willard, Ph.D., Biological Safety Officer
at 353-1877.
The online form
is divided into 7 pages. To progress to the next page, click on
the "Save and proceed to next page" button at the bottom
of each page. If you wish to return to the previous page, do not
use the "Back" button on your web browser because any
information that you entered on that page will be lost. To return
to the previous page, click on the "Save and return to previous
page" button at the bottom of the screen. Please complete all
parts to each question.
If you have
questions about a particular section of the form, click on the colored,
underlined link for more information. A box will open up that will
give additional information or instructions for completing that
section of the form.
If you need
to stop filling out the form and continue it later, click on the
"Save and Proceed to Next Page" button at the bottom of
that screen. Each time this button is clicked, any information that
was entered is saved to the database. Even if you have not filled
out the whole form, you may exit the system at this time or go to
another website without losing any information.
When you wish
to view, modify, or finish your registration form, the saved document
may be accessed by choosing the "Search rDNA Applications"
option from the main page and signing in using your name and Principal
Investigator access code.
Once you reach
the end of the form, a report will appear containing the information
that you entered. Please print this out and keep it for your records.
Points to
Remember
- Write out
or explain acronyms
- Consult the
NIH Guidelines to identify which category (and subcategory) your
research falls under.
- Write your
technical summary so that it can be understood by a non-scientist.
- When indicating
which biosafety level will be used, remember to include the location
where the research will take place. This location may be subject
to an inspection prior to committee review.
- Leave adequate
time for the committee to review and discuss your project, and
for the paperwork to be processed.

or classification of recombinant DNA molecules, the NIH Guidelines
for Research Involving Recombinant DNA Molecules (NIH Guidelines)
may be a valuable source for you. This manual may be found on the
Internet through the Biosafety in Research page (www.biosafety.msu.edu)
under the recombinant DNA section. For a hard copy of this document,
please call the Biosafety Staff at the Office of Radiation, Chemical
and Biological Safety at 355-0153.
Another source
of information is the series of links throughout the form, which
can be clicked on for access to additional information about that
portion of the registration.
